As part of the registration process, there are documents that need to be uploaded. These include:
- Proof of Residency (Utility bill, Rental/Lease agreement (1st page only), mortgage statement, etc)
- Immunization Records
- Birth Verification (Birth Certificate, Consular Report of Birth Abroad, Hospital Certificate, Immigration Document, etc.)
- Legal guardianship or custody papers, if applicable
If these are scanned or photographed ahead of time, they can be uploaded during the registration process.
There are also documents that need to be filled in, such as Consent for Medication at school, Attendance Guidelines, Internet Use Agreement, etc. These are available as fillable forms below or on the right side of this page. Once filled out, save them on the computer/tablet/phone and they can be uploaded at the end of the registration process.
